For patients from other allergy offices who are already on immunotherapy:
After new patient visit and review of records, we might agree to continue to administer immunotherapy serum provided by another allergy office.
We provide this as a COURTESY SERVICE. As such, please be advised that we will not bear any responsibility for your serum in case of accidental damage while in our care (broken bottles, damage due to temperature from failed refrigeration equipment etc.). The office providing your serum has to agree to replace the serum for free in case of accidental damage. Alternatively, you as the patient can sign an agreement not to hold our office liable and replace the serum at your own cost. Refusal to sign this agreement means that we will not provide the injection service, NO EXCEPTION. We will not accept your serum until we have this agreement, so please do not bring it with you to your first appointment.
