Your Insurance Coverage
Allergy & Asthma of Springfield, LLC is a provider for most major insurance plans, and provides insurance billing. You are responsible for charges that are not covered by your insurance at the time of your appointment.
What to Bring to Your Appointment
· Your insurance card, drivers license, or valid state ID
· List of prescriptions and/or over-the-counter medication, including dose and frequency
· List all known medication allergies
· Information about your medical and surgical history
· Recent x-rays, labs, or relevant records if outside of Mercy
· New Patient Form (filled out, see bellow)
· All minors without parents or legal guardians must have a signed minor consent form (see bellow)
· Your co-pay is due the same day (we accept Visa, MasterCard, Discover, Cash, Check)

What to Expect
New patient appointments typically include a thorough medical history, exam, and allergy testing. The time and cost of this visit will vary based on your symptoms and the testing that your allergist recommends. The evaluation usually takes 2-3 hours.
Based on the evaluation, the doctor will formulate a diagnosis and recommend treatment options. Education regarding your diagnosis and treatment plan is an important part of your initial visit. We will work with you to create a treatment program that provides the most effective therapy for your unique medical issues. Our goal is to arrive at a treatment plan which allows you to take control of your asthma or allergy symptoms.
If you are being evaluated for allergies or asthma, we recommend you stop taking antihistamines (such as Benadryl, Claritin, Allegra, Zyrtec, Xyzal, hydroxyzine, cyproheptadine) at least 3 full days before your appointment. If you are unsure if you are on an antihistamine, please call our office. Do not stop your other medications (particularly asthma or blood pressure medications).
